It turns out one of the springs in the clutch plate broke and then lodged itself against the pressure plate. Then the pressure build up and apparently blew out the slave cylinder. So long story short, $1200 later I have a brand new clutch!
Broken clutch..
I was driving, came to a stop light, and then that was it. I was in neutral, I could not shift into any gear. We pushed my car over to the side of the road and called a tow truck.

WOW, the spring sheared in half, crazy. I have heard and seen some that have popped out, but they didn't shear I don't think. That sucks.

I'd bet it did simply pop out of the disc, any damage to the spring itself was probably caused after it was fliying around inside the bell housing.
